APPENDIX A: INFORMED CONSENT FORM

1. Introduction
You are being invited to take part in a research study on data integrity and management within the Directorate of Veterans Affairs (DVA) of the UPDF. Before you decide whether to take part, please read the information below and feel free to ask the researcher any questions you may have.
2. Purpose of the Study
The purpose of this study is to assess current data integrity and security practices within the DVA, and to use this understanding to design and validate a secure framework for data integrity and management systems.
3. What Your Participation Involves
If you agree to take part, you will be asked to complete a questionnaire, take part in an interview, or participate in a focus group discussion lasting approximately thirty to sixty minutes. Some participants may also be invited to review and comment on the proposed secure framework during the validation phase. There are no correct or incorrect answers; the researcher is interested in your genuine experience and opinions.
4. Voluntary Participation and Withdrawal
Your participation in this study is entirely voluntary. You may decline to answer any question, and you may withdraw from the study at any time without giving a reason and without penalty or loss of any benefit to which you are otherwise entitled, including your employment status or access to DVA services.
5. Risks, Discomforts & Benefits
Participation involves minimal risk. Some questions ask about data handling weaknesses or security incidents you may be aware of, which could cause mild discomfort. You are free to skip any question that makes you uncomfortable. While there is no direct financial compensation, your responses will directly contribute to designing a secure framework intended to help the DVA protect veteran records and improve service delivery.
6. Confidentiality & Data Protection
All information you provide will be kept strictly confidential. Your name will not appear on the questionnaire or in any final academic report. Data is stored in secure, encrypted systems accessible solely to the researcher.
